PSE, also called hepatic encephalopathy, is frequently diagnosed in patients with advanced stages of liver cirrhosis. It is caused by the accumulation of toxic breakdown products (e.g. ammonia) in the blood and characterised by mental and neurological changes, even leading to coma.

>> Decreasing the ammonia level in the blood 
Lactulose lowers excessive blood ammonia levels by inhibiting the formation of ammonia in the bowel due to growth of biomass (e.g. Bifidobacteria). Moreover, accumulation of ammonia in the blood is reduced by promoting the diffusion of ammonia from the blood into
the intestinal lumen. This leads to the excretion of ammonia with the stool.

Lactulose inhibits the rise of blood ammonia levels by reducing ammonia formation in the bowel and its transfer into the blood. It has been considered the gold standard in PSE therapy since 1966.
